Coordinated Veterans’ Care (CVC) Program
The CVC Program focuses on proactive, team-based care led by your GP. Through structured care planning, regular monitoring, and coordination with allied health providers, we aim to reduce hospitalisations, improve health outcomes, and ensure veterans receive personalised, continuous medical support.
At Gordon Family Practice, we proudly support eligible veterans through the Coordinated Veterans’ Care (CVC) Program. This Department of Veterans’ Affairs (DVA) initiative is designed to improve the management of chronic conditions and enhance quality of life for veterans living with ongoing health issues.
Eligibility & Enrolment
The program is available to eligible DVA Gold Card holders and certain White Card holders with chronic conditions. Your GP will assess eligibility based on medical complexity and risk of hospitalisation.
