Chronic Disease Care

Chronic disease management to support patients living with long-term health conditions such as heart disease, diabetes, asthma, chronic kidney disease, and osteoporosis. Chronic illnesses require structured monitoring, medication management, lifestyle support, and preventive care to reduce complications and maintain quality of life. Our GPs develop personalised care plans aligned with Australian clinical guidelines to ensure coordinated, evidence-based, and ongoing support tailored to each patient’s needs.

Our approach to chronic disease care focuses on early intervention, regular health reviews, and proactive risk reduction to prevent hospitalisation and disease progression. Through GP Management Plans, Team Care Arrangements, and collaboration with allied health professionals, we provide integrated and patient-centred care.

Heart Disease & Cardiovascular Care

Heart disease is one of the leading causes of serious illness in Australia. Early detection, regular monitoring, and lifestyle management significantly reduce the risk of heart attack and stroke.

  • Blood pressure monitoring
  • Cholesterol testing and management
  • Cardiovascular risk assessment
  • ECG referrals (when required)
  • Medication review and optimisation
  • Lifestyle advice for heart disease prevention

Diabetes Management

Diabetes is a chronic condition that affects blood sugar regulation and increases the risk of heart, kidney, and nerve complications. Ongoing monitoring and structured care improve long-term outcomes.

  • Blood glucose and HbA1c monitoring
  • Individual Diabetes Management Plans
  • Medication and insulin management
  • Complication screening
  • Kidney and eye health monitoring
  • Lifestyle and nutrition guidance

Chronic Kidney Disease (CKD)

Chronic kidney disease may progress silently without noticeable symptoms. Early diagnosis and blood pressure control are essential to protect kidney function.

  • Kidney function blood tests (eGFR)
  • Urine protein testing
  • Blood pressure management
  • Medication safety review
  • Diabetes-related kidney monitoring
  • Specialist referral when required

Asthma & Emphysema (COPD)

Chronic respiratory diseases can significantly affect breathing and daily activities. Structured monitoring and correct inhaler use help prevent flare-ups and hospitalisation.

  • Asthma action plans
  • Spirometry and lung function testing
  • Inhaler technique assessment
  • COPD and emphysema management
  • Smoking cessation support
  • Flu and pneumonia vaccination advice

Osteoporosis & Bone Health

Osteoporosis weakens bones and increases fracture risk, particularly in older adults. Early screening and treatment reduce the risk of falls and serious injury.

  • Bone health risk assessment
  • DEXA scan referrals
  • Calcium and vitamin D guidance
  • Fall risk evaluation
  • Medication management
  • Weight-bearing exercise advice

A chronic disease is a long-term medical condition such as heart disease, diabetes, kidney disease, asthma, COPD, or osteoporosis that requires ongoing management.

Common signs include chest pain, shortness of breath, fatigue, dizziness, and high blood pressure. However, heart disease can develop without symptoms, which is why regular screening is important.

Most chronic conditions should be reviewed every 3 to 6 months, depending on severity and stability.

A GP Management Plan is a structured treatment plan developed by your GP to coordinate care, set health goals, and organise referrals for chronic disease management.

Eligible patients may access Medicare-supported GP Management Plans and allied health services under Team Care Arrangements.

Early diagnosis reduces the risk of severe complications such as heart attack, stroke, kidney failure, fractures, and hospitalisation.
